MEDICAID WAIVER PROGRAM

Indiana Medicaid Waiver (Home & Community‑Based Services)

​

Indiana’s Medicaid waiver lets eligible seniors and adults with disabilities get care at home instead of in a nursing facility. It’s designed to keep people safe, independent, and supported in their own communities. 

​

Two Waivers After July 1, 2024

​

Indiana reorganized its long-term care waivers:

​

  • Health & Wellness Waiver – for individuals 59 and under

  • Indiana PathWays for Aging Waiver – for individuals 60 and older
    Both are part of Medicaid’s home- and community-based services (HCBS). â€‹

​

Who Qualifies?

​

To be eligible, a person must:

  • Be aged, blind, or disabled

  • Meet nursing facility level of care

  • Live (or plan to live) in a non-institutional, HCBS-compliant setting

  • Have income at or below 300% of the SSI maximum (parent income is disregarded for minors) â€‹

​

What the Waiver Can Cover​

​

Services are tailored to each person’s needs and may include help with activities of daily living (bathing, dressing, meals, medication reminders), housekeeping, transportation, companionship, and memory care supports. 

​

Structured Family Caregiving (Get Paid to Care)

​

Under the waiver, Indiana offers Structured Family Caregiving (SFC)—a model where a participant lives with a primary caregiver who provides daily support and may receive a stipend.
